In order to receive MindOH!’s Discipline and Life Skills Series through one of our corporate underwriters, the following requirements must be met:

1. Commit to regularly using the program for all discipline referrals
2. Review monthly utilization and performance reports distributed by MindOH! and make changes accordingly
3. Works with MindOH! to submit to sponsor on a quarterly basis
4. Works with MindOH! to review program impacts on academic performance
5. Answer email or return calls within 24 hours
6. Incorporate MindOH! into the School Improvement and Discipline Management Plans
7. Have New School Datasheet completed and returned to MindOH! at least 1 week prior to the Initial Strategic Meeting
8. Will allow us to publish results and allow positive media interaction
9. At least 5 staff members involved with MindOH! will complete a short feedback survey per semester
10. Will have all initial meetings scheduled within 2 weeks of receiving funds
11. Complete pre-implementation tasks within 2 weeks of the initial strategic meeting, including: completion of the liaison and technical checklists, and incorporation of the program into the disciplinary process
